Recheck Appointments
Clients who have not been to the Veterinary Neurological Center in more than 1 year should fill out this form prior to their recheck appointment. This will help to ensure our records are current. Please submit online, email or fax the form to the VNC, or give it to the receptionist upon arrival.

Clients who have scheduled an appointment may fill out this form prior to their appointment to expedite the registration process. Current lab work (CBC, Chemistry, Urinalysis) performed within 10 days is required for any patient undergoing an anesthetic procedure at the VNC. Client Correspondence Forms:
You may submit your request online to obtain refills for a particular medication. Once you have submitted the form, your medication will be ready for pickup within 2 business days at the Veterinary Neurological Center or your local pharmacy (depending on your selection). Many common pharmaceuticals and their uses are listed in the Pharmaceutical Glossary. Additionally, some medications will need to be compounded at special compounding pharmacies rather than retail pharmacies. Use this form to report the details of your pet’s seizure(s) to your doctor(s) at the Veterinary Neurological Center. The doctor(s) will analyze this information and if you have questions or if changes are necessary to your pet’s treatment plan, we will contact you shortly. Clients may use this form to submit questions, comments, and updates about their pet. Images may also be uploaded, including pet photos for inclusion on our Patient Pictures page. Client Resource Forms:
The Client Guide are provided to all clients following their first appointment at the VNC. The information contained in the Client Guide is meant to help you communicate with your neurologist, neurology resident and their support team, and guide you through the process of understanding and caring for your pet’s neurological issue. If you have any questions or concerns that are not addressed in the Client Guide, please email, or call us Monday-Saturday, 9am-5pm.
The purpose of this form is to inform you of the risks associated with anesthesia, sedation and occasionally hospitalization, which are inherent for dogs with shorter muzzles (brachycephalic). Not all of these problems may apply to your pet but are apart of brachycephalic syndrome. If you have any specific questions or concerns, please bring them up so that they can be discussed with one of our neurologists
These are the directions for the care and use of the collection system for a urinary catheter to be used if your pet is sent home with an urinary catheter. If you have any questions or concerns regarding your pet’s urinary catheter, please call us Monday-Saturday, 9am-5pm.
